Below are common forms used at LCS. Some forms require membership login.

Accreditation is a must for any private institution that wishes to remain competitive and maintain a legitimate reputation. 

Any and all official changes within your school must be reported in writing using the Change of Status Form. 

By joining this community of Christian schools, you join school leaders, thinkers, and experts who are willing to help you with challenges in reaching your community.

Teachers can track their CEs and hours and also monitor the completion of any and ALL teacher credential requirements.

Teacher Certification Program provides professional certification for qualified educators desiring to pursue a ministry in Christian education

Preschool Directors must complete quarterly self insepctions of their own centers. These should be given to your inspector.

Accredited schools must submit the annual accreditation survey and progress report to document school improvement efforts.

Preschool inspectors use this form to conduct on-site inspections for all preschools that are under our Religious Exempt Program.
